FAIRBURY – Jefferson Community Health Center was one of 11 hospitals from across the country asked to share its story in early February at the American Hospital Association’s AHA Health Forum Rural Health Care Leadership Conference in Phoenix, Ariz. Erin Starr, chief quality officer, presented “Improving Harm Across the Board” on behalf of JCHC.
“JCHC has accomplished great quality of care as evidenced by zero surgical site infections, zero catheter-associated urinary tract infections, zero hospital-acquired blood clots and just one central line-associated infection in the last two years,” Starr said. “We have also accomplished great strides in reducing readmissions.”
Readmission is noted when a patient who has been hospitalized is readmitted to a hospital for any reason within 30 days.
Among efforts JCHC had made in these areas are:
*Quality Council review of all falls, readmissions, medication errors, and fall-outs on measures;
*Central line protocols were revised by Infection Preventionist;
*Participating in CAPTURE Falls program with University of Nebraska Medical Center, implementing post-fall huddles, and revising fall assessment tool;
*Medical staff engaged in investigation process on all core measures fall-outs;
*Front line engagement through the development of nursing protocols, including nurse-driven foley removal protocol and Nursing Council utilized for input on policies/protocols;
*And established care transitions team, implementing coaches for COPD and Heart Failure and in-depth reviews by pharmacist and dietitian.
“We are honored for the opportunity to present what we have accomplished and what we have learned along with other hospitals throughout the nation,” Starr said. “Quality and safety remain our top priority.”
